We, couple, asked people on gas station and found a french family going to Cannes. They took us to highway gas station near Savona, where were opened door from the station to the Savona town.

The traffic is not bad, we used a combination of one guy asks at the door of the autogrill and the second tries with a thumb up, eventually got a car using the thumb to Pisa, but I would try asking the foreign cars in person to maximise chances since Italians simply wont stop🥲

the spot is just a gas station...theresl was a guy working there and a sign saying NO AUTOSTOP, but literally the first van i asked to drove to to Arenzano, about 30km away

For heading west there is a gas station here that is open 24 hours. Zoom in and check out the satellite view. Directly north of the station there is the entrance to the highway that catches traffic coming through the station as well as from the main road. Just stand at this point with a sign for Varazze or Savona and get off at any big service station along the way. If you are downtown, to get to the gas station get on bus no 1 and just say to the driver "Autostrade Genova Ovest" or hand him a note. I had poor luck after 6pm (which is why the 24 hours station was handy) but caught a ride quickly in the morning. Don't make a sign for too far west or the locals will think you're crazy. Baby steps. We got a ride to Tortona here, however we were very lucky. There must be better spots on this road - but this road looks like a good one for the autostrada.

Should be OK for getting a ride west. But we used it for getting a ride EAST - city-hitching into Genova centre - and it worked very well.

The spot is good but not one stopped. It is right before the highway next to a shell gas station. There is plenty of room for the cars to pull over on the left side. There is also shelter underneath a building that crosses the on ramp. One thing that made me nervous was the police station but they didn't give me any problems. I have found Italy very hard to hitch in.

For me this was a good spot to go to France as there was a lot of traffic (we've been waiting for 30 minutes). But you're still in Italy, so make sure that the city on your sign isn't that far (Imperia or Savona). It's a good spot to get on the highway and as you'll know, once you're their the sky is the limit.

Good spot to go to France, but step by step and not directly.
I asked to people who stopped at the gas station if they went to Savona or Imperia, I just had to wait 15 minutes.
I was then taken to a gas station inside the highway and immediately found a car to Marseille.
